Proof of Professionally Licensed Business / Shop:

5th Avenue Studio Supply only sells to licensed shops for all Piercing and Tattoo Supplies. If you are a new customer, we will verify your shop, to include calling the shop directly, internet search, and / or verification through State Business Entity look up service.

If an order has been placed by an unauthorized person or entity, the order will be cancelled, the created account on 5-ave.com will be deleted, and you will be charged a $25.00 Fee for processing of a fraudulent order.

COD:

All COD (Cash on Delivery) orders will be charged an additional COD fee of $13.00. COD payment must be presented at the time of delivery by cashiers check or money order only.  To have personal or business check COD privileges, you must receive prior approval.

If a COD order is returned to 5th Avenue Studio Supply for any reason, COD privileges will be revoked and all future orders must be pre-paid.  All shipping costs incurred by 5th Avenue Studio Supply for returned packages will be billed back to the customer and payment of incurred charges must be paid immediately and prior to any future orders being shipped.

Shipping:

All orders are sent UPS Ground, unless another form of shipment is requested.  All orders MUST include a street or route address (We cannot accept a P.O. Box number for UPS shipments).  All orders are subject to shipping and handling charges.  5th Avenue Studio Supply stocks over 600,000 items for shipping the next business day.  We pride ourselves on prompt shipment.

Next Business Day Shipping:

All orders received prior to 3:00 pm (MST) are processed immediately and are guaranteed to be shipped the next business day.  However, we will always try to ship them the same day as received.  Most manufacturers take 3-5 days to ship even if the items are in stock.  We know that you want your order fast and will do our best to deliver.

Return Procedure:

Returns are only available for Body Jewelry.  Due to health concerns, tattoo and piercing supplies, to include ink, needles, and grips, may not be returned.  

For Jewelry returns you may call Monday through Friday, 1-800-385-3005 , 8:00 AM - 5:00 PM MST for immediate return authorization.  Items being returned should be packed securely in appropriate sized and sturdy cartons.  A copy of our invoice must be enclosed with your merchandise and a letter listing the item number, reason for return and specify replacement, exchange or credit.  Please insure your shipment, 5th Avenue Studio Supply is not responsible for any lost or damaged packages.  Most returns are processed within 2 working days of receipt.  This procedure has been designed to handle your return in an accurate and timely manner.  Please help us by following these procedures.
